Pet health record template

The one page an emergency clinic, sitter or boarding kennel asks for first: identity, vaccines, medications, conditions and who to call. Fill it in here, print it, or download it as text. It saves in your browser between visits.

How this tool works

  1. Fill in the fields you know; leave the rest for after the next vet visit.
  2. Print the page or download the summary as a text file.
  3. Update it after every vaccine, prescription or weight change.

Why these fields

They are the questions asked at an emergency intake desk and on boarding forms: species and breed, weight, microchip, vaccine status, current medications, known conditions and allergies, diet, and contacts for the regular vet, an emergency clinic and a backup person. Keeping them on one page means you are not scrolling through a camera roll under stress.

Questions about this tool

This tool is general information, not veterinary advice. It runs in your browser; what you type stays on your device unless you choose to save it to a PetVitale account. Always confirm schedules, doses and decisions with your veterinarian.
